This Privacy Policy describes how we collect, use, disclose, manage and process your personal data that you provide to us when you submit in a registration form to make an appointment with us through our online booking appointment portal available at https://rsc-book.online/ and/or when you provide personal data to us through Google Forms when you personally visit our service centres in Singapore.

You will also be required to make a health declaration at https://rsc.center/sg for contact tracing purposes when you personally visit our service centres during the outbreak of the Coronavirus disease 2019 (Covid-19). Such information will be collected and disclosed solely for the purposes of contact tracing during the period of the Covid-19 outbreak.

By completing and submitting the registration form to us, you are accepting the terms of our Privacy Policy, and thereby consenting to our collection, usage and disclosure of your Personal Data as described in this Privacy Policy. When you make an appointment with us through our online booking appointment portal, you will be asked to provide the following personal data: 

  • your First and Last name;
  • salutation;
  • telephone number;
  • preferred language;
  • email address;
  • last four alphanumeric characters of your NRIC / passport number / identity card number;
  • residential address;
  • details of your visits to our website, the activities you engage in when accessing our website and the resources that you access on or via our website.
